The bottom line of what I'll abbreviate as SS, is that it is supposedly better for performance -- presumably specifically in endurance activities -- because it effectively provides a slow release glucose source that is low GI (doesn't spike glucose much), doesn't elicit a significant insulin response, and therefore doesn't suppress fat burning. Some of which may be true, and some of which is not supported by the evidence provided by the company. </div>

Jimmy Moore, and now his wife Christine Moore, are not really interested or seeking optimal health. Jimmy has long given up on weight loss, and it matters not what the actual weight is on the scale at this point, he's behind the ball position on January 1, 2004 (when he began Atkins) in so many ways. He has a serious eating disorder at the root of all of this ... something he shows no desire to address. Very sad for him and those who love him. </div>

Still, he inexplicably preys on desperate obese people seeking solutions or excuses. He does this to feed an insatiable need for accolades from others, and to live a higher life than he otherwise would be (like multiple cruises each year!).<br>

He makes carbohydrate restriction-based plans -- I don't care what the next fad (and that's what it is!) incarnation is -- look bad.<br>

If keto or LCHF or Atkins or whatever has worked for you, and continues to work for you, go for it! I'm NOT anti-LC. But do not spread junk science. Do not follow and elevate the likes of Jimmy Moore who is FAILING eating that way. Go forth and share your own story if you want. Or not. Probably not a good idea to turn it into your primary source of income. If you disagree with my assessment of "failing", step back, imagine if he's any <u>fill-in-the-blank</u> spokesperson for some diet you think is bull$#!t and see if you wouldn't agree. He is the one delegitimizing your successful way of life in the eyes of others. He is the one adding to all the confusion by spouting nonsense like steak is chocolate cake (true story).<br>

Carbohydrate restriction plans could gain more mainstream traction if its promoters, ESPECIALLY doctors and researchers, stuck to the facts and what the science shows. It's just that won't make those folks rich, and the accepted science will be nowhere near the exaggerated claims you've heard repeated in your echo chambers. </div>

In addition to blogging on several topics raised during Stephan Guyenet and Gary Taubes recent <b><a href="https://youtu.be/vA3QavPp1Ho" target="_blank">appearance on the Joe Rogan Experience podcast</a></b>, I've decided to bump some of the past content on this blog relevant to some topics discussed and/or research studies mentioned. I'll include a little commentary with each.</div>

At around the 12 minute mark, Stephan makes a great opening play, and then gets distracted or flustered into fumbling the football. Stephan asks what it is about the food environment, etc. that causes obesity, and distills it down to the simplest of questions (paraphrase):</div>

Q: What is the most fattening diet on the planet?<br>A: Human junk food.</blockquote>
<div style="text-align: justify;">
Nail. Head. Boom. He then calls out some numbers from <a href="http://www.stephanguyenet.com/references-for-my-debate-with-gary-taubes-on-the-joe-rogan-experience/" target="_blank">the research summary he posted on his blog in advance of the podcast</a>. The study I discuss in this blog post is the one included in "#52" that he's discussing in the podcast. Joe seemed perplexed by this idea that we couldn't replicate the junk food fattening with sugar or even fat alone. Joe seems fixated on junk food = sugary "bullshit diet". He gets very confused, despite Stephan laying it out fairly well. Give just sugar/refined carb or give just fat, and the rats don't get AS obese as they do when you give them a mixture of sugar/fat and FLAVOR in human junk foods. Yes, Joe, soda is just sugar and is a junk food, but junk foods are also things like donuts, cookies and ice cream or pizza, fries and Doritos. Carb + fat + flavor. <br><br>While the CAF rats are indeed rats, this study does a fairly good job in demonstrating that it's not just the macros. Four diets: Standard Chow (mostly grains, 12% fat) , Low Fat Refined (35% sugar, 10% fat) diet, High Fat diet (45% fat, as lard and soybean oil) and CAF -- a wide assortment of junk foods from sugary breakfast cereals, to Doritos, to pork rinds, to wedding cake. If it's carbs and insulin, why don't the rats on the SC get fat? If it's just refined carbs, why don't the sugar rats get fat? If it's just the fats, why don't the lardos get fat? These two groups of rats did gain more weight, but -- as Stephan mentions -- they don't gain a lot more, and more importantly, they don't overeat energy. The CAF rats, meanwhile, consumed over 30% more food/calories and got a whole lot fatter. <br><br>Gary's Response: We're looking at this wrong. Calories are the wrong way to look at this. </div>

This post should perhaps have come first, but it has taken a while to look deeply at the data for the primary outcome -- total energy expenditure measured by doubly labeled water -- and related outcomes of intake and resting energy expenditure.</div>
<div style="text-align: justify;">

In this study, all participants were paid to participate, AND provided free food for ~8 months. Said food was professionally and meticulously prepped, weighed, measured, individualized to provide each subject with some pretty exact caloric level and macronutrient composition. The test phase (in other words, the study proper) involved <b><i>maintaining a consistent weight</i></b> (to within ±2 kg), for a considerable length of time: 20 weeks, assessed at the midpoint and the end.<br>
<br>
In the previous study, a roughly 300 calorie/day increase in TEE was observed during 4 weeks of isocaloric feeding, producing no weight loss. This was essentially dismissed as "not enough time to show weight loss" {paraphrase} .... So in the current study, the average change in TEE masks the often wild swings of <b style="font-style: italic;">up to 2000 calories per day </b>between 10 week maintenance time points. <b style="font-style: italic;"> </b>If this increase (or decrease) in expenditure is truly real, the subjects would HAVE to have adjusted intake accordingly over the course of several weeks in order to maintain consistent weight. </div>

This post is about the massive discrepancies between intake and TEE data (also compared with REE), and the inexcusable near-dismissal of these discrepancies in the journal article.</div>

Either the DLW-TEE data is (for reasons that can be discussed) unreliable/inaccurate, or the intake data is unreliable. If the TEE data is problematic, how can a result of a difference of means of 250 cal/day be justifiably defended as significant/meaningful, when we see no such difference in intake? If the intake data is problematic, then how can we conclude anything from a "diet comparison", because significant deviations in caloric intake would render the strict macro compositions totally irrelevant. </div>

Either way, the researchers failed to address this glaring problem with the results study. Further, the discrepancies in these two data sets are so egregious that it is impossible that both can be considered accurate and reliable. It has to be one or the other. <span style="color: blue;"><b>Based on the data for Resting Energy Expenditure (REE), the needle points to the intake data as the more reliable measure.</b></span><br>

Ultimately, </div><div style="text-align: justify;"><ul><li>randomization to the various test diets occurred after weight loss (PWL)</li><li>all subjects lost weight on the same 45% Carb / 30% Fat / 25% Protein diet targeting a weight loss of 12% ± 2% initial body weight.</li></ul>The researchers do not appear to have made many adjustments in the weight loss phase to produce a more uniform weight loss. Rather than 12% ± 2% (from 10% to 14%) losses, the actual outcome was roughly 10.5% ± 5% (actual range from 5.6% to 16.0%. </div><div style="text-align: justify;"><br></div><div style="text-align: justify;">To repeat, the amount of weight loss is irrespective of the test diet. Everyone had baseline insulin measures taken, everyone was put on the same weight loss diet. Thus we have an "accidental" test-within-a-test of the CIH/TWICHOO from these "early" results.<br>
<br>I offer these scatter plots for the105 subjects who successfully completed the study and for whom complete data for insulin measures and energy expenditure were available at all time points.<br>

The Carb-Insulin Hypothesis (aka TWICHOO) predicts that weight loss will vary inversely with insulin levels: The higher the insulin levels, the lesser the weight loss. The Run-In Phase data supports no such relationship (indeed, if anything, absolute weight loss was greater for those with higher baseline insulin measures.<br>
<br>
Meanwhile, differences in weight loss are easily explained by variation in caloric deficit during the calorie restricted Run-In due to coarse estimation of baseline energy expenditure (vs. rigorous measure).<br>
<br>
This post expands on some relationships of baseline (BSL) and post-weight loss (PWL) measures as observed during the weight loss portion of the Run-In Phase.</div>

Regarding this post, focusing on the <span style="color: red;"><b>OUTRIGHT FAILURE TO IMPLEMENT THEIR STUDY DESIGN AS STATED</b></span>, I stress the following from the previous paper.</div>

Virtually every stipulation leading up to the study, the Abstract and Full Text of the journal article, and continuing through to Ludwig's November 28, 2018 response to Kevin Hall in <i>BMJ</i>,<i> </i>has stated that the MAIN purpose of the Run-In phase was to achieve 12% ± 2% weight loss. The study was intended to test the so-called Carb-Insulin Hypothesis during 20 weeks of weight-stable maintenance on diets of varying carbohydrate content.<br>

<b>OUTCOME:</b><br>
<blockquote class="tr_bq">
With only 10 weeks of 60% calorie restriction during Run-In, the subjects who completed this phase averaged 10.5% weight loss (std.dev. 1.7%), ranging from only 5.6% up to 16.7% weight loss. This doesn't change much for analyses of just those completers for whom complete energy data are available (same mean, SD = 1.5%, range 5.9% to 16.0%).</blockquote>

He has apparently been reading (in fits and starts, in other words, probably not really reading) obscure books about obscure cultures from long, long ago. Mind you, that in five years plus of arduous and comprehensive research put forth in <i>Good Calories, Bad Calories</i>, there was no mention of the <a href="https://en.wikipedia.org/wiki/Yaghan_people" target="_blank">Yahgan</a> people he's about to discuss. One wonders why not. Heck, this is right in his time period of excellence for nutritional research and reporting! (<a href="https://amzn.to/2vlnKnN" style="font-style: italic;" target="_blank">Uttermost Parts of the Earth</a><i>, </i><span style="font-size: xx-small;">this is to a 2007 version of a book Taubes states was published in 1948</span>). But alas, no Yahgan <span style="font-size: xx-small;">(I also checked Wikipedia's alternate spellings)</span>, among the conventional-wisdom-challenging by Taubes circa 2007.<br>

Several years ago, Dr. Michael Eades wrote the following post: <a href="http://www.proteinpower.com/drmike/saturated-fat/abcs-big-meal-propaganda/" target="_blank">ABC’s big meal propaganda</a>. Sadly, the video is no longer available. It involved subjects consuming a GIGANTIC meal of 6000 calories, after which blood was drawn two hours later. This "after" blood was very cloudy, and the technician holds this up and identifies the source of the cloudiness as fat. The meal was deep fat fried mac&cheese, a bacon cheese burger quesadilla and fries and an ice cream smothered giant cookie. Yes, high in fat and carbs, but favoring the starches. While they pointed finger at just the saturated fat -- it was 187g of saturated fat == it's not too much of a stretch to estimate this meal contained around 300-350g total fat or more.<br>

The AMA classified Obesity as a disease in 2013. Five years later we've made little to no progress, likely because we've got "experts" pontificating and arguing over semantics and false dichotomies. The false dichotomy of Disease vs. Choice is right out of whatever master playbook teaches that if you keep the "masses" arguing, they might not notice you're full of it. It's really the only explanation I can think of for the circular arguments made with nary a tinge of irony on board straight faces.<br>

As to Mark's contention that "it's like we're almost made for it" ... there's a table at his own link that shows that as a species outside of infancy and pregnancy, compared to rats, we're actually "not so much". Mark knows this. Staying in therapeutic ketosis as an adult (not this nutritional gimmick nonsense) is rather difficult for most people. I've been reminded recently that for some, even a leaf of spinach or the taste of artificial sweeteners can be enough to banish one from planet Keton<span style="color: #666666;"> <span style="font-size: xx-small;"><b>(<a href="https://twitter.com/LouSchuler/status/986614395278016513" target="_blank">coined by</a> @LouSchuler on Twitter). </b></span></span>I won't even quibble<span style="color: #666666;"><span style="font-size: xx-small;"><b> (<a href="https://twitter.com/pdd18/status/987746384404254721" target="_blank">again from conversation on Twitter)</a> </b></span></span>with evidence that we aren't "born in ketosis". </div>

As the recent <a href="http://www.stephanguyenet.com/the-second-nusi-funded-diet-trial-has-arrived/" target="_blank">Gardner-led, partially NuSI funded study demonstrated</a>, modern American humans -- for various reasons -- have difficulty adhering to extreme diets. This is not evidence against the *potential* for various diets to be effective, nor is it evidence that following such diets is harmful for the long term. But it is evidence that as a dietary prescription of sorts, neither is going to garner much success in the general population. But one last thing before I go on this point. I've mentioned this before, and I'll mention it again. We have BILLIONS of humans who have thrived on very low fat diets throughout their lives, for generations. We just don't have anything near this for low carb diets, let alone keto versions. In his article, Sisson says the following:</div>

Look at the full text I provided. This was a case study on THREE young adults with GLUT-1 deficiency. Two who were normal weight at baseline lost a little bit of weight in the form of fat. One who was underweight at baseline gained weight: this individual grew 1.5 cm indicating she wan't fully grown at baseline, and gained more fat than weight (gained 1.5 kg overall, 2kg fat, thus lost 0.5 kg FFM). The authors mention that the results here are in contrast to those seen in children treated for epilepsy because of full bone mineralization at baseline. <b><span style="color: red;"> Perhaps, at the very least, there should be extreme caution for growing children or teens to embark on a keto diet! </span></b>I see no reason to trust Mark Sisson on the long-term appropriateness of any diet. </div>

He's not healing anything. He's playing around with whatever healthy function he has left for social media likes and podcast downloads so he can sell keto stuff to earn affiliate dollars. First up -- who else? LOL -- Jimmy Moore. Fresh off his second 7-day fast in three weeks, Jimmy continues focusing on all the wrong things instead of actual improvements in things known to improve health. Apparently low carb is/wasn't enough for Jimmy Moore, so he adopted an extreme version of a keto diet. This worked swimmingly for a year until he stopped reporting his weight at the end of the year. He rapidly regained the 80 re-lost pounds and embarked on his first extended fast in the fall of 2015. That went so well he tried (unsuccessfully) to forego food for the entirety of January 2016, only to balloon up even further. Forget the scale, I won't even post images, you've seen them all. He is a profoundly unhealthy man despite cherry-picked biomarkers that appear to indicate otherwise. </div>

Jimmy Moore is obsessed with measuring ketones (everywhichway, apparently, but up the bum -- is an anal ketone test far behind? LOL) and blood sugar. Unless you are using insulin to control diabetes and/or concerned about ketoacidosis, or are on a therapeutic ketogenic diet for something like epilepsy, incessant glucose and (especially) ketone monitoring is pretty meaningless and probably neuroses-inducing in many. </div>

BOTTOM LINE: When ANYONE (other than an IDDM) doesn't eat for a few days, their blood sugars will go down and their ketone levels will go up. So big whoop that this happened to Jimmy Moore. </div>

Summary:<br><br>In the Women's Health Initiative, a 2017 analysis of the energy density of baseline diets, stratified to quintiles, revealed several correlative components of an energy dense diet (comparing the top quintile (Q5) to the bottom (Q1). The energy density, in kcal/gram of food, nearly doubled in Q5 vs. Q1 corresponding to a nearly 50% increase in caloric intake. The dietary components correlating with this are:<br>
<br>
<ul>
<li>Fat: Fat content is by far and away the greatest contributing factor to consuming an energy dense diet. Absolute intake more than doubled (2.5X) from Q1 to Q5, and comprised roughly 90% of the increased caloric intake.</li>
<li>Animal Protein: As protein intake from plant sources remained relatively constant, animal protein intake increased by 40% resulting in an increase in total protein of 25%. Therefore animal foods are a considerable contributor to increased energy density of the diet.</li>
<li>Added Sugars: Added sugar comprised 11% of caloric intake across the quintiles, however absolute added sugar increased almost 50%. </li>
<li>Carbohydrate: Total carbohydrate intake is not associated with increased energy density of the diet. Increases in added sugars were offset by reduction in other carbohydrates so that absolute intake remained constant. Thus as a percentage of the caloric intake, carbohydrate declined significantly.</li>
<li>The macro percent composition of the least energy dense diets was P/F/C of 18/26/59, while the percents were 16/45/41 for the most energy dense diets.</li>
</ul>
<div>
The near doubling of energy density is associated with an (adjusted) roughly 25% increase in the risk of developing diabetes.</div>
