Why We Get Fat ... Lessons from a Cafeteria Rat

BUMP! In addition to blogging on several topics raised during Stephan Guyenet and Gary Taubes recent appearance on the Joe Rogan Experience podcast , I've decided to bump some of the past content on this blog relevant to some topics discussed and/or research studies mentioned.  I'll include a little commentary with each. At around the 12 minute mark, Stephan makes a great opening play, and then gets distracted or flustered into fumbling the football.  Stephan asks what it is about the food environment, etc. that causes obesity, and distills it down to the simplest of questions (paraphrase): Q:  What is the most fattening diet on the planet? Guyenet v. Taubes ~ Topic 1: "Just 10-20 Calories a Day"

U nless you've been living under a "rock" in the world of nutrition, you're aware that Stephan Guyenet "debated" Gary Taubes on the Joe Rogan Experience podcast. It took me several days broken up to watch/listen to the whole thing. In a nutshell, it was Science & Substance (Stephan Guyenet) versus Stories & Slander (Gary Taubes). Rather than summarize, there were several points made that I'd like to address. In no particular order. I'll include this intro with links to prior topics as this series progresses.

Insulin Treatment in Diabetes ~ Why Does It Often Cause Weight Gain?

S ummary: Diabetes, whether Type 1 or Type 2, is a dysfunctional, wasteful metabolic state.  As a result, an uncontrolled diabetic either uses or loses more energy than their non-diabetic selves would otherwise use.  As such, the untreated diabetic is essentially "underweight" compared with the body weight that the same energy intake would produce were they not diabetic. There are differences in endogenous insulin production between the two types of diabetes.  In Type 1, there is effectively no insulin production.  In Type 2, there is usually elevated basal insulin production, but a relative deficiency in acute insulin secretion, specifically an impaired early insulin response to glucose (GSIS).  The absolute or relative insulin deficiency results in the following to a greater or lesser degree: Excessive lipolysis resulting in an increased cycling of the Triglyceride/Fatty Acid cycle.   Impaired suppression of glucose production in t...

Tom Naughton Doubles Down on Kooky Kalorie Klaims for Fat Head Kids

Preface: I have to say this.  Sorry, not sorry.  If you have children, and you want to help them with nutrition and health as they grow, teach them about food and teach them how to cook.   Get your own weight and health under control (if it's a problem) so that you can pass down good habits to your children.  Or do nothing.  None of my business.  But whatever you do, do not buy your child a copy of Fat Head Kids or let them within 50 yards of a showing of the Fat Head Kids movie if it ever comes out.  Put a block on your Hulu or whatever streaming service it is eventually on.  I cannot recommend more strongly against this. Every time I have watched snippets of some video of Tom talking about this book, or read the preview content on Amazon, I just get utterly creeped out by the whole thing, and Tom Naughton specifically.  Books for children should not include shirtless pictures of the middle aged author who goes on about boy boobs....

Productive Why-ning

SUMMARY: Back in the early days of the Asylum, there was a commenter who repeatedly fixated on "why do we overeat".  Eventually I coined the phrase "why-ning" for this questioning. This post is about how seeking external whys in the scientific literature is most likely a futile effort.  Science can provide easy answers, it turns out, to how to eat to normalize weight.  Putting that into practice in the *real world*, is a whole 'nother issue!!! If you struggle with weight ... if you're putting all manner of effort in anyway ... this post is a suggestion to alter your "why-ning".  From unproductive questions of why, to more productive, individual and personal questions of yourself as to why. There are a number of reasons for this, but one of them has been that I start some train of thought or down some research rabbit hole, and just never get around to finishing a post to my liking ... then things move on. B ump! One would think that the calorie or energy-balance denialists would have gotten a clue by now, but apparently this post needs bumping.   If you make it through this post and still believe calories are some non-entity, that somehow insulin or any hormone can trump energy balance, I don't know what to say. Are the Super-Obese the Ones Who Are Metabolically Adapted?

Introduction The current theme of metabolic research seems to have settled on physiological changes, post-weight-loss in the obese, that set the person up for almost assured failure and regain.  It has been reported that the super-obese (usually a term for BMI>40) tend to have REEs that are higher than would be predicted by standard, generally accepted models such as Harris-Benedict or Mifflin-St.Jeor.  This is both what was seen in the Biggest Losers "before" weight loss, and was reported in comments on this blog by Dr. Yoni Freedhoff, who measures REE in the obese he treats.   In order for a super-obese person to fully reverse their obesity, they will need to lose large amounts of weight -- 100, 200 or more pounds.   To do so, most will need to dramatically cut calories to lose at a rate that is motivating enough to sustain a fairly lengthy time period.  Even "fast" weight loss, reported by many, and especially gastric bypass patients, occurs over...

The Latest Metabolic Adaptation Study

A quick post here just to get it out there, and I'm already a couple weeks behind the curve.  Oddly haven't heard too much about this, or perhaps it's because I've lost the will to deal with the community that would jump on this?  Who knows :-) There is a new NIH funded study out of Columbia (Rosenbaum & Leibel) published this month:   Models of Energy Homeostasis in Response to Maintenance of Reduced Body Weight I'm going to limit this post to just one of the three experiments conducted: In 17 subjects (14 women, 3 men) weight stable energy expenditure and body composition  was determined following a period of 6-8 weeks of weight stability.  They were then put on an 800 cal/day diet until they had reduced their weight by 10% of initial weight.  This took 7-13 weeks, after which their food intake was increased to achieve weight stability at the 10% reduced weight for 6-8 weeks.  Energy expenditure and body composition were measured her...

How Does the Carbohydrate-Insulin Model aka TWICHOO, Explain the Obesity Epidemic?

A short post ... something I hit on again in the process of writing another one. Below is a plot of the absolute amounts of protein, carb and fat that Americans claimed to consume from 1965 through 2011 (adapted from Hite & Economic Litigation Consultants, 2015 , blog post ).   All but 1965 is NHANES data, but I'll even "give them" the inappropriate toss-in of the 1965 Sesame-Street-song data point. I've properly lined up the timelines for prevalence of BMI > 25 curve   (that scale -- not shown -- goes from just over 45% to just under 75%. ).   This is the same curve repeated by Adele Hite and so, so many others -- often immediately after chastising Ancel Keys for equating correlation with causation (keep ignoring thqt  he didn't do that  with his infamous plot) -- in an attempt to implicate the Dietary Guidelines in the obesity epidemic.    Even IF one could pin the obesity epidemic on an increase in carbohydrate consumption,...

It's TOTAL Energy Expenditure that Matters, and RMR Doesn't Necessarily Predict It!

Post Overview: In the context of weight loss, maintenance or gain, eventually all roads lead to the acceptance that *calories matter* ... you must be in deficit, balance or surplus respectively. In the wake of the recent The Biggest Loser Regain study , there has been a lot of doom-and-gloom reporting, led by Gina Kolata and Sandra Aamodt  in the New York Times .   I've distilled the results down to the bare bones: Summarized from Table 1 from Fothergill et.al.   14 Contestants (6 Men , 8 Women) The "alarming outcome" was that resting metabolic rate was reduced as might be expected, but seemingly remained suppressed and even further declined despite significant re-gain (70% re-gain/loss for the mean) after 6 years.   However the TEEs -- the TOTAL energy expenditures for the day -- tell a different tale.   These were measured over a roughly two week period by doubly labeled water in free-living conditions. The TEEs remained high at 30 weeks...

Dateline:  March 26, 2016 Helloooo Inmates!! A quick update, but first a sincere thank you to each and every one who participated in my 40 Day Lenten Lentil Fast post.  As of this morning -- two months after publication -- we stand at just over 2700 comments.  No that's not a typo.  Who knew all I had to do was stop blogging?    It fills my heart to have such great interaction here at any time.  It especially filled it these past two months, months that have been more tumultuous than time-constrained, and some of the most difficult of my life.  So THANK YOU, and especially for much needed laughter along the way!  What more is there to say? My Gut Feeling on Microflobesity Was Right

This post brought to you by Duck Dodgers' arrogance.  If there ever were an example of what is wrong with science in the Incestral Health Community, it is the phenomenon that is "Duck Dodgers" and his comments here.     A seemingly non-controversial post -- on how the US Dietary Guidelines were never any radical departure from the way humans around the globe have eaten for thousands of years -- went viral, at least in part because of Duck's return.   What rubs me personally the wrong way about this person (or persons, though I've been told it is only one person who posts with that ID in comments here so I operate on that assumption) is his response to any challenge to "How Food Enrichment Made Us Fat, Diabetic, and Chronically Diseased" otherwise known as the "Iron Food Enrichment Hypothesis" .  Despite doubling down on the whole fortification thing at the off-the-grid web rental only around a month ago, this guy comes on here and plays...

Over on Twitter I was made aware that Dr. David Ludwig is coming out with a new book in January (HT Steven Snyder @NoGimmicksNutri) mentioned in this Time piece:   You Asked: Should I Count Calories?    The book is to be called Always Hungry   (affiliate link if you're going to buy it anyway!)   This is a similar title to the NYT editorial Ludwig co-wrote with NuSI's VP of Research last year, discussed in this post.  I also have a friend who attended an obesity conference recently at which Ludwig presented much of the same ideas.  It is repackaged TWICHOO (Taubes Wrong Insulin-Carbohydrate Hypothesis Of Obesity) , and what is most galling about all of this is that much of Ludwig's own research counters this ... For today.  Just thoughts.  No references.  Also I will use the term theory, it sounds better, and frankly in the semantics of scientific progression, we've been there for a while now with Adiposopathy (I'll capitalize it for emphasis) anyway. Since offhandedly throwing out the term Adiposopathy in response to a query as to what I consider to be the cause of CVD/diabetes, etc.  -- collectively known as "metabolic diseases" and sometimes "chronic diseases" -- there's been one question raised a few times:  How and/or why do fat cells get "sick".  I think I found the perfect illustration ... take it away Lucy and Ethel ...

I really thought this was a joke, and a silent hat tip goes out to the person who brought this to my attention so's that I don't get them on anyone's bad list for contributing to such a post here at the Asylum.  But OMG . How Many Calories Does Mayo Have? ... now that Mark Sisson is whipping up wonderful mayo, I can get to work on the backlog of tuna salad, pea salad, and deviled eggs I’ve been denying myself. ... I thought I’d enjoy some tuna salad with two heaping spoonfuls of Mark’s zippy mayo—call it research for  lower calorie  meal plans for my new business, the FatBurn Factory.  With the addition of a few capers, instant lunch! But I need to know for, for the sake of my patients, ...    WHAT IS THE CALORIE COUNT OF THIS QUICK AND EASY MEAL  . The Obesity "Paradox"

From his very first foray into the abyss that is "horrible" nutrition science, right on up through today, one thing has remained consistent for Gary Taubes.  Sure, the alpha glycerol phosphate idea where you can't store fat without dietary carb was blasted to smitherines by his own reference decades before the advance for GCBC was paid out.  Nevermind that billions of white rice eaters countered his insulin spiking notions.  Don't even notice the man behind the fructose curtain, a mirage known as the non-insulin spiking carb.   As any good man of science will tell you, Gary Taubes insists that any hypothesis (and thus any supporting mechanisms) must be able to explain all of the observations.  I agree.  Too bad he doesn't hold his own hypotheses up to even a modicum of his own standards, or perhaps we might be congratulating Peter Attia and Gary on their successful opening of the $40Mil Newcastle-in-America Diabetes Clinic.  But instead, NuSI i...

Just Eat Less - Move Moore, Jimmy

Another timely bump to go with the Three-Year "Update" for Jimmy Moore.  Seems he is receiving the expected backlash from the recent Paleof(x) CONference, and his pre-game post failed to stave off the gasps and criticisms.  Andreas "look at all the fat people on this cruise" Eenfeldt has just posted a short recap video from the event.  The comments on his blog are interesting to say the least.  Seems to have a strange thing going with Jimmy, kind of like Tom Naughton.  They both post unflattering images, etc. of their "friend".  The absolute worst light to show Jimmy in is a profile, and yet this is what his "friend" Andreas chose to use a lot of.  Eh ... their dysfunction.
